Prognostic Value of Microvascular Obstruction and Infarct Size, as Measured by CMR in STEMI Patients

2014

The aim of this study was to evaluate the value of microvascular obstruction (MO) and infarct size as a percentage of left ventricular mass (15%LV), as measured by contrast-enhanced cardiac magnetic resonance, in predicting major cardiovascular adverse events (MACE) at 2 years in patients with ST-segment elevation myocardial infarction reperfused by primary percutaneous coronary intervention. Individual data from 1,025 patients were entered into the pooled analysis. MO was associated with the occurrence of MACE, defined as a composite of cardiac death, congestive heart failure, and myocardial re-infarction (adjusted hazard ratio: 3.74; 95% confidence interval: 2.21 to 6.34). IS% LV >= 25% w…

Microvascular Obstruction in ST-Segment Elevation Myocardial Infarction: Looking Back to Move Forward. Focus on CMR

2019

After a myocardial infarction (MI), despite the resolution of the coronary occlusion, the deterioration of myocardial perfusion persists in a considerable number of patients. This phenomenon is known as microvascular obstruction (MVO). Initially, the focus was placed on re-establishing blood flow in the epicardial artery. Then, the observation that MVO has profound negative structural and prognostic repercussions revived interest in microcirculation. In the near future, the availability of co-adjuvant therapies (beyond timely coronary reperfusion) aimed at preventing, minimizing, and repairing MVOs and finding convincing answers to questions regarding what, when, how, and where to administe…

Deep learning architectures for automatic detection of viable myocardiac segments

2021

Thesis abstract: Deep learning architectures for automatic detection of viable myocardiac segmentsAccurate myocardial segmentation in LGE-MRI is an important purpose for diagnosis assistance of infarcted patients. Nevertheless, manual delineation of target volumes is time-consuming and depends on intra- and inter-observer variability. This thesis aims at developing efficient deep learning-based methods for automatically segmenting myocardial tissues (healthy myocardium, myocardial infarction, and microvascular obstruction) on LGE-MRI. In this regard, we first proposed a 2.5D SegU-Net model based on a fusion framework (U-Net and SegNet) to learn different feature representations adaptively. …

Prognostic Value of Initial Left Ventricular Remodeling in Patients With Reperfused STEMI

2019

Abstract Objectives This study sought to establish the best definition of left ventricular adverse remodeling (LVAR) to predict outcomes and determine whether its assessment adds prognostic information to that obtained by early cardiac magnetic resonance (CMR). Background LVAR, usually defined as an increase in left ventricular end-diastolic volume (LVEDV) is the main cause of heart failure after an ST-segment elevated myocardial infarction; however, the role of assessment of LVAR in predicting cardiovascular events remains controversial. Methods Patients with ST-segment elevated myocardial infarction who received percutaneous coronary intervention within 6 h of symptom onset were included …
